By submitting this form, you agree that MeridianLink may retain and use your contact information and uploaded media. You may revoke your consent at any time by clicking on the "unsubscribe" link in the footer of any email, or by contacting the MeridianLink directly at HR@Meridianlink.com. You also agree that the photo and information you provide as part of the MeridianLink Virtual Mosaic (not including contact information) may be used by MeridianLink on its Website, social media channels and onsite. You also agree that the information you provide (including your email address) can be shared with (include other entities if any) for sales and outreach purposes.

Privacy Policy
I hereby give my employer MeridianLink, Inc. consent to videotape and photograph my image to be used in the following ways:
-Company intranet— accessible by employees only
-Company accounts and website, social media channels, and onsite— viewable via the public world wide web
-Client/customer newsletters, presentations, emails, and printed materials
-Employee newsletters, presentations, emails, and printed materials
-Advertisements, publicity, commercial and any other business purposes
I understand that this permission also applies to any recordings, videotape, or photographs that I submit to the company as part of company events and activities. I further acknowledge that this permission extends to individuals that I have included in these submissions; including children and/or minors only for use on Company intranet and internal communications without my express written permission.
I further understand that no special compensation will be provided to me for use of my image, and that I may not be informed in advance of the specific use of my image.
I hereby release and agree to fully and unconditionally defend, indemnify and hold harmless MeridianLink, its officers, agents and employees from any claims, liabilities, or other damages that may arise from my recordings/images initial or subsequent use.
I acknowledge that this authorization is continuous, and I must complete and submit a new image release form and indicate that I wish to no longer participate in order to withdraw my consent, understanding that certain data may have already been released prior that action and not all released materials may later be redacted.
